1. Registered Exporter Form Which Issued By Competent Authority In Confirmed Country Of Origin By Representatives Of The Republic Of Indonesia Abroad Include:
A. Company Profile Registered Exporter And/or;
B. The Legality Of The Permit Published By Agency Local In The Country Of Origin;
2. Statement Letter From Exporter Country Of Origin Stating That:
A. Non B3 Waste As Industrial Raw Materials That Exported:
1) Does Not Orginate From Landfills Activities;
2) Is Not A Trash And Is Not Mixed With Trash;
3) Is Not Contaminated With Hazardous And Toxic Materials And Waste (b3); And
4) Is Homogenous.
B. Willing To Take Responsibility And Receive Back Non B3 Waste As Industrial Raw Materials That Has Been Exported In Terms Of Not In Accordance With The Statement As Referred To In Number 3 Letter A;
